The following is a list of co-managed schemes available in the North West Lancashire area.

|Blackpool, North Lancs||Easy to manage, convenient for Patients, releases hospital appointments. Stable Glaucoma discharge Patients discharged to named optometrist, who performs baseline assessment of IOP, Field and disc. These are compared to Hospital’s last readings, which are sent to optometrist with target IOP etc for comparison. If stability confirmed, then Patient stays with optometrist, but referred back if moves outside certain criteria|

|Preston, Chorley, and South Ribble||Optometrist takes images through dilated pupil. Images are then sent to the Diabetic screening group via internet, and technical failures are currently assessed by optometrist on site.|
|Minor Eye Condition Service (Managed by First Choice Eye Care)||Blackpool, Fylde and
|Patients can be seen with red or other eye related queries into optometrists who agree to participate. Patient is assessed and course of treatment suggested, referred back to GP if appropriate onto hospital eye service, or simply suggestion of action to patient. GP gets full report. Referral can be made by GP, self referral or optometrist.|

|Low Vision Aids (LVAs)||Blackpool, Fylde, Wyre, Preston, Chorley, and South Ribble||Simple Aids can sometimes benefit Patients with early problems that they could not otherwise access. Optometrists can use a “selected” range of aids that benefit many Patients, at relatively low cost per Patient. No Charge to patient|
|Cataract – post-operative 4 week check||Preston, Chorley, South Ribble, Blackpool, Fylde and Wyre.||Patients who have had cataract surgery can be seen for their 4 week post operative check by accredited optometrists. This provides quicker discharge back to the patient’s own optometrist and reduces activity in secondary care outpatients.|
|Glaucoma Referral Refinement||North Lancs (including Lancaster/Morecambe/Carnforth)||
Patients who’s IOP is assessed at being more than 21mm Hg can have a follow up undertaken using applanation tonometry to ensure appropriate referral to secondary care if necessary.
